Overview
You do not need insurance to get healthcare in Houston. The city has one of the strongest safety-net healthcare systems in the country — anchored by Harris Health System, Legacy Community Health, and dozens of federally qualified health centers (FQHCs) that serve anyone regardless of ability to pay, insurance status, or immigration status.
Federally qualified health centers use a sliding-fee scale — you pay based on your income. If you make very little, you may pay nothing. These clinics provide primary care, dental, vision, behavioral health, and pharmacy services. They exist specifically so that cost is never a barrier to basic healthcare.

Harris Health System is the public hospital system for Harris County. It operates Ben Taub Hospital (Level I trauma center), Lyndon B. Johnson Hospital, and 18+ community health centers across the county. Harris Health serves anyone — insured or not. Financial assistance is available through their eligibility program.
Federally Qualified Health Centers (FQHCs) are nonprofit clinics funded partly by the federal government. They must serve everyone and charge on a sliding scale. Major FQHC networks in Houston:
- Legacy Community Health — 40+ locations across Houston. Primary care, dental, vision, behavioral health, HIV services, pharmacy.
- Avenue 360 Health & Wellness — multiple locations, strong behavioral health and substance abuse programs.
- Hope Clinic — serves the Asian-American community with multilingual staff. Primary care, dental, women's health.
- El Centro de Corazon — bilingual (English/Spanish), multiple eastside locations.
Sliding-fee scale: At FQHCs, you bring proof of income (pay stub, tax return, or self-declaration) and household size. The clinic calculates your fee. At 100% of poverty or below, most visits are $0-$20. Even without any documentation, you will not be turned away.

Step 1: Find the closest clinic

Step 2: Call ahead
Call the clinic to schedule a new patient appointment. Ask about: walk-in availability, what documents to bring, interpreter services, and which services they offer on-site (dental, vision, pharmacy).
Step 3: What to bring to your first visit
- Photo ID (any form — driver's license, passport, consulate ID, school ID)
- Proof of income if you have it (for sliding-fee calculation)
- Proof of address (utility bill, lease, mail)
- Insurance card if you have one (Medicaid, Medicare, private)
- List of current medications
- If you have none of these, go anyway. They will help you.
Need insurance?
- Medicaid: Apply at yourtexasbenefits.com. Covers children, pregnant women, seniors, and people with disabilities.
- Marketplace plans: healthcare.gov — open enrollment November-January, but qualifying life events allow enrollment anytime.
- Harris Health financial assistance: Apply at any Harris Health location. Covers hospital and clinic visits for uninsured Harris County residents.
