C.A.R.E. Community Health Worker Initiative

Overview

The C.A.R.E. Initiative deploys trained community health workers into predominantly Hispanic and Latino communities in Southwest Houston. Launched in July 2020, the program connects residents to wraparound resources including health care, food assistance, housing support, and other essential services. We welcome everyone in our community regardless of background or ability to pay.

IM Houston

For over 60 years, Interfaith Ministries for Greater Houston has brought people of diverse faith traditions together for dialogue, collaboration, and service to the most vulnerable in our community. IM Houston operates Texas's largest Meals on Wheels program (delivering 1.5 million meals annually to 4,300+ homebound seniors), resettles refugees from 40+ countries, coordinates thousands of disaster and community volunteers through Volunteer Houston, and fosters interfaith understanding through dialogue programs, youth leadership, and cultural exchange.

Magnolia Multi-Service Center

The Magnolia Multi-Service Center is a City of Houston facility on Capitol Street serving the Magnolia Park neighborhood on Houston's near east side. Magnolia Park is one of Houston's oldest Latino communities, and the MSC is positioned to serve a population that includes many Spanish-speaking families, recent immigrants, and long-established households. Fifth Ward Multi-Service Center

The Fifth Ward Multi-Service Center is a City of Houston facility serving one of Houston's oldest and most historically significant African American neighborhoods. Located on Market Street, the center provides Fifth Ward residents with direct access to health screenings, public benefits navigation, social services, and community programming. The facility reflects the city's commitment to delivering municipal resources in communities that have historically faced disinvestment and limited access to public services.
